A patient/his or her next-of- kin/agent/lawyer can obtain medical report from Unit Medico Legal, Jabatan Rekod Perubatan, Wisma Rekod (opposite Maternity Hospital) or send it an application via mail.

Patient or his/her next-of-kin/agent/lawyer must bring along:-

Application Form/Letter of Application. (PER ML-102)
Copy of patient's clinic appointments card/ hospital bill/ discharge note.
Copy of patient's Identification Card
Insurance / KWSP / PERKESO form (If require)
Consent letter given by patient (required for agent, his/her next-of-kin).

Expected duration of completion for a report is in 4 weeks.
